Management of Pancreatitis Due to Pancreatic Injury in Cytoreductive Surgery

Abstract

Objective

The experience with six patients who developed pancreatitis after cytoreductive surgery and intraperitoneal chemotherapy was reviewed, and based on this data, new treatment strategies were formulated.

Background

Patients with peritoneal carcinomatosis from non-invasive adenocarcinoma of the colon or appendix can benefit long term from aggressive intraperitoneal treatments. In this approach, extensive peritonectomy procedures are combined with chemotherapy infused into the peritoneal cavity in the early postoperative period. The surgery involves a stripping of cancer from the surface of many vital structures, including the pancreas.

Methods

The record of 255 patients who underwent these treatments were reviewed, and those who had clinical evidence for postoperative pancreatitis were identified for special study. The clinical features of the surgery and treatment of the postoperative pancreatitis were critically evaluated.

Results

All patients who developed postoperative apncreatitis had a visible tear in the capsule of the pancreas as a result of surgical trauma. No patients who did not have such an injury developed pancreatitis. Pancreatitis lead to extensive postoperative complications in all six patients, with a mean hospital stay of 50 days.

Conclusions

Trauma to the pancreas that causes a visible tear in the capsule is to be avoided at the time of cyroreductive surgery. If this occurs, wide drainage of enzyme-rich material must be instituted. When a tear in the capsule of the pancreas occurs, it must be assumed that pancreatitis is a possible complication of the postoperative course that would require aggressive preventive measures.
